Insurance/Payment

Effective February 1, 2015, Richard A Berg PC will no longer accept insurance for payment. The practice has opted out of participation in Medicare, effective April 1, 2015.  Insurance claims will not be filed. We will not comply with requests for additional information from insurance companies beyond providing a copy of the professional report with the permission of the individual.

The aeromedical evaluations are being conducted per FAA or employer requirements and not by medical necessity. They are considered occupational/fitness-for-duty assessments or are being done to pursue recreational aviation activities.

All major credit cards, checks, and cash are accepted for payment which is due at the time of service unless prior arrangements have been made.  

Appointments

• Your appointment is a period of time set aside especially for you. If you find you will not be able to come to your appointment, please notify Dr. Berg (phone, email, or text) you will not be coming at least 48 hours in advance. There are, of course, exceptions for emergencies.

• Late cancellations ( less than 48 hours prior to the scheduled appointment time) will be subject to a fee equivalent to the total fee quoted for the appointment.

• “No-show” individuals will be required to pay the full evaluation fee for the missed appointment (credit card or cash only) before another appointment will be scheduled.
